Sub and amp integrated in one side of the boot. That looks good actually. Wouldn't mind having that.

Would require fibreglass fabrication to make the custom enclosure. Here is another one

Does it sound good tho? Would you get the busting wrattling noises if its in fibreglass. Plus would it be a great inconvenience to get to spare tire.
It does look quite good though. Especially the first one.

The fibreglass enclosure would still have fill in it like an MDF box and would be sufficiently thick enough. Like everywhere else soundproofing is the key. It would be moulded to fit snugley into the area with a couple of millimeters clearance. It would be removeable so wouldn't effect access to the tyre well and give you extra boot space if needed.
Cost is the factor. A custom fibreglass box costs alot more than your standard 19-22mm thick MDF enclosure. I'd say a box like that would set you back around $500 mainly because of the labour involved.
Rearward firing subs sound best in hatchbacks but not to say this kind of design would sound bad and its a great space saver. You still have full use of the boot moreso than a traditional box.
Another option would be a sub enclosure moulded into the tyre well. This is quite popular on Integras. The sub would fire up. You might need to build up the floor of the boot to get a big enough enclosure and cost would be even more than the side enclosure.

The U.S. Ipod adapter is priced around $200 USD PLUS install - from the looks of the new 06 Civic cable insatll it's a few hours work to get this bugger in...bummer it didn't come standard!

Doors are a bad place to put your crossovers. Mine are up inside the back of the dash on each side where they can't rattle about or get moisture in them. You can put them anywhere though as cable length won't effect their performance. A lot of competition cars have them in the boot on display.

The price on the ipod adapter is a lot steeper than I expected. I can understand about the installation time though since the head unit would have to be pulled out to attach it to the back.
The aftermarket ipod adapters are around A$150 so you'd be better off changing the head unit for something decent like Pioneer or Alpine and getting their ipod adapter.
